RN Utilization Manager / RN Case Manager
Good Life Home Care, an established, family-owned and Medicare certified home health agency seeks a Registered Nurse with case management and/or management experience to join our growing organization. This exciting opportunity will appeal to a highly professional, organized, motivated and compassionate individual with at least 2 years of clinical work experience within the home care service industry.
POSITION DESCRIPTION
o Responsible for the day to day coordination and oversight of admissions, patient management and interdisciplinary communication to promote seamless delivery of care
o Conducts start-of-care and ongoing reviews of assigned cases to ensure optimal and cost effective utilization of agency resources
o Conducts real-time reviews of patient documentation to ensure that clinicians have captured a comprehensive representation of each patient's needs, goals and clinicians' interventions, and meets regulatory requirements
o Provides support and mentorship to assigned clinicians, including participation in orientations and skill checks, developing opportunities for continuing education, performance improvement and enhancement of internal systems and processes that free clinicians to focus on quality patient care
JOB REQUIREMENTS
o Current RN License in the State of CA
o 2-4 Years work experience as a nurse within the home care industry
o Broad-based clinical knowledge, including chronic disease management, infusion services and wound care
o A high-level understanding of home health agency regulations and documentation requirements, including CMS Conditions of Participation (COP's) and OASIS-C1
o Ability to integrate nursing, therapy and ancillary service resources
o Current Driver's License and Auto Insurance
o Excellent written, verbal and interpersonal communication skills

Estimated Salary: $20 to $28 per hour based on qualifications.
