The PT shall support CHW/Dominican Hospitals strategic plan and the goals and direction of the quality improvement / process improvement activities and patient satisfaction.

Experience:

Minimum of three years of experience as a Physical Therapist in physical disabilities setting, preferably in acute care, acute rehabilitation or home health Knowledge of physical therapy theory, treatment, and evaluation Previous home health experience preferred.
Must be an experienced computer user PT IIAdditional Experience and other Requirements:
Currently employed as an agency PT at 0.
5 FTE status or greater Employed minimally for 3 months in Dominican Home Health (or 1 year previous Home Health experience in another HH agency) Demonstrates competency in OASIS admission assessments Meets criteria and performs Home Health Case Management duties Position

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
