Harbor SLS is seeking Direct Support Professionals/Lead Staff to assist, teach, and serve individuals with developmental/intellectual disabilities to be as independent and integrated into the community as possible.

DIRECT SUPPORT PROFESSIONAL/STAFF LEAD POSITION JOB DESCRIPTION Supported Living Services (SLS) provides support to adults with developmental disabilities who wish to receive support to live successfully in our community.
Qualifications / Education:
1 year of experience in a supported living environment or similar 18 years old or older Authorized to work in the US Qualifications include:
Satisfactory oral and written communication, math and computer skills A high school diploma or its equivalent A minimum of one year experience with adults who have developmental disabilities (or similar experience) is strongly preferred The ability to maintain First Aid & CPR Certification Position requires a valid California driver's license, a good driving record and a registered and insured vehicle Department of Justice level criminal background check Familiarity with SLS programs Requirements:
Personal Assistance:
Ability to assist the individual with activities of daily living including bathing, toileting, catheterizing and other self-help skills.
Lifting / Physical Demands:
Ability to lift 50 or more pounds frequently Challenging Behaviors:

As a Housemate you will be working an agreed upon schedule and eligible for benefits.
Concurrent enrollment as an IHSS provider may be required.
Work Schedule:
Scheduled shifts are dependent on the needs of the person receiving support, in addition to the needs of the Facilitator.
Staff must be available weekdays, evenings, weekends and holidays, as well working a rotating on-call schedule.
Duties /
Responsibilities:
Assists Facilitator with scheduling and shift coverage.
Responsible for coverage if a substitute staff is not found Schedules and leads monthly team meetings and ensures staff attendance Monitors daily shift notes for completion and content Collects paperwork, turns into office by deadline Completes monthly service reviews and quarterly safety checklists Responsible for review and upkeep of household supplies Updates home binders at least monthly, insuring all information is up to date Picks up medications, reviews for accuracy, updates medication logs and training documents as needed and at least monthly Posts monthly activity calendar, records all planned activities and meetings as well as posting Harbor correspondence Ensures the safety of the person supported in their home and the community Assists person receiving support in achieving their personal, behavioral, educational and/or vocational goals as identified in the Individual Service Plan (ISP) Assists with household duties such as cleaning, cooking and household management (may include yard upkeep) to maintain the household in accordance with the guidelines determined by the person receiving support.
Provides necessary support for person receiving services to meet personal needs Provides skills training to person supported, team members and new staff Maintains current First Aid and CPR certification Participates in all mandatory meetings and trainings Responds to crisis in accordance with Harbor policies and training Assists person receiving support with medical needs and appointments Provides supervision and support to person receiving services in their home and the community Communicates and coordinates services with Facilitator and Harbor management team Develops and maintains written documentation as requested Complies with Harbor's goals, policies and procedures Makes decisions based on Harbor's Mission and Value Statement Completes all other duties as assigned by Facilitator These duties and responsibilities are considered to be essential to the position under the terms of the Americans with Disabilities Act (ADA); these designations may change at any time according to the needs of the program.
